Out-Patient Support Coordinator

Position Summary The Out-Patient Support Coordinator provides long term mental health case management for persons with serious and persistent mental illness.

The overarching goal is to provide appropriate, empathetic, non-judgmental and respectful care urgently to individuals identifying any sort of problematic mental health while ultimately helping the client to increase their quality of life and decrease hospitalization.

• Support complex clients in a community environment
• Coordinate client care with other service providers
• Collect assessment data, plan, implement and evaluate needs of assigned caseload
• Review client’s history, diagnosis, coping patterns, level of function, including physical, medical and cognitive deficits
• Assess current social networks and needs related to environmental supports and cultural context
• Screen for concurrent addiction issues
• Assess for capability for treatment and support decisions
• Complete informed consent processes
• Evaluate basic immediate needs, strengths, deficits, past and present involvement with services and capacity for independent living
• Measure for readiness to engage in Recovery planning and facilitate the Recovery planning process
• Engage the client in the planning process related to personal goals, values and lifestyle
• Reassess client’s needs on an ongoing basis
• Assess family support and education needs
• Reviews and monitor symptoms and medication
• Engages with the crisis intervention system as needed and appropriate What we’re looking for
• Bachelors or Master’s in a recognized Social Sciences program
• Valid registration with the relevant college/governing body in Ontario, such as the Ontario College of registered Psychotherapists (CRPO), or in pursuit of relevant credential is mandatory
• 3 years’ experience in mental health in a similar role
• Demonstrated experience working with concurrent psychiatric disorders, dual diagnosis, mood disorders, Axis 1, Post-Traumatic Stress Disorder and Major Depressive Disorders.

• Knowledge of addictions along a continuum of care
• Excellent verbal and written communication skills
• Organized, meticulous, technical and detail oriented
• Professional, collaborative and thrive in a fast-paced environment
• Valid Ontario Driver’s License, access to own vehicle and business insurance
• Ability to transport client as appropriate or needed
• Engaging, collaborative and creative, astute to clients’ needs with the ability to adapt and be flexible
