SUMMARY OF THE POSITIONThe Community Support Coordinator (CSC) is responsible for coordinating and providing support to people with mental health issues living in the community.

Support activities may include any of the following: assistance in finding and maintaining housing, crisis intervention, prevention and support, recovery planning, life skill development, education and advocacy.

The Community Support Coordinator will perform work activities in a manner consistent with the mission and values of Thresholds Homes and Supports Inc.

The Community Support Coordinator reports to the Program Manager.

HOURS OF WORKThe CSC will work thirty-five (35) hours per week and is required to work flexible hours including evenings and be part of the on-call rotation.

This is a temporary full time position till March 31, 2021.

REQUIRED QUALIFICATIONSPost-secondary degree or diploma in human services / human sciences or related field.Minimum three (3) years’ experience working in a community mental health setting.Demonstrated ability of being a highly effective communicator with strong interpersonal and teamwork skillsExperience in supporting individuals experiencing acute crisis and where possible taking action that is consistent with the consumer’s crisis plan of actionPrioritizing and organizing work is second nature and able to make recommendationsSound knowledge of community resources and the ability to liaise with community agencies, generic services, mental health professionals, specialized health care, recreation and leisure opportunities and vocational programsAbility to advocate and liaise with housing providers, landlords, boarding homes, shelter operations and operators of other housing facilitiesProficient in the use of Word, Excel, electronic record keeping and data collection systems as well as OCAN and CRMSStrong assessment skills and demonstrated ability to assess complex situations and make appropriate decisions independentlyCertification in First Aid/CPR, Non-Violent Crisis Intervention and suicide risk assessment preferredMust have a valid Ontario driver’s license, relevant insurance and use of a reliable vehicleAble to successfully complete and provide a current Police Vulnerable Sector Check
